    The Union Cycliste Internationale (UCI), which governs professional cycling, worries that it might. At a meeting on January 31st,2025, the UCI recommended that its bosses ban the use of carbon monoxide by riders on medical grounds. The UCI has even requested the World Anti-Doping Agency (WADA) to decide whether repeatedly breathing in carbon monoxide should be considered doping (兴奋剂).

    That a poisonous gas might be performance-enhancing is less surprising than it sounds. Hemoglobin (血红蛋白) attaches to oxygen to transport it throughout the body, but CO attaches to haemoglobin more strongly than oxygen does, preventing oxygen transport. While too much exposure can lead to asphyxiation (窒息), smaller doses can cause mild lack of oxygen. Extended lack of oxygen stimulates the bone marrow (骨髓) to increase red blood cell production, which offers benefits for endurance athletes. More red blood cells enhance the blood's oxygen-carrying capacity, enabling athletes to perform better over long distances.

    It indeed works in practice. In 2020 a group of researchers led by Walter Schmidt at the University of Bayreuth published a paper examining the effects of CO on bicycle athletes. They found that small doses given five times a day for three weeks led to a roughly 5% increase in the amount of haemoglobin in their blood. Levels of haemoglobin found in red blood cells is an important statistic for endurance athletes.

    Even more appealing, though, is the fact that the gas has legal uses. Indeed, CO is routinely used to keep track of riders' fitness. Moreover, all the equipment and supplies necessary to abuse the gas are already widely used.
